Abstract

Orta Kulak T¸berk¸lozu: Olgu Sunumu ÷ZET Orta kulak t¸berk¸lozu geliºmiº ¸lkelerde nadir gˆzlenen bir hastal˝kt˝r. Diðer orta kulak hastal˝klar˝yla kar˝ºabilmesi nedeniyle tan˝ konmas˝ g¸Át¸r. Standart tedaviye yan˝t vermeyen kronik orta kulak enfeksiyonlar˝nda ay˝r˝c˝ tan˝da orta kulak t¸berk¸lozu d¸º¸n¸lmelidir. Erken tan˝ ve tedavi, komplikasyonlar˝ ˆnler ve hastal˝ð˝ s˝n˝rlar. Baz˝ ilerlememiº vakalarda cerrahiye gerek kalmadan antit¸berk¸loz tedavi etkili ve yeterlidir. 47 yaº˝nda erkek hasta kliniðimize 1 y˝ld˝r sistemik ve lokal antibiyotik tedavisi ile d¸zelmeyen kˆt¸ kokulu sol kulak ak˝nt˝s˝, iºitme kayb˝ ve 10 g¸nd¸r y¸z¸n¸n sol taraf˝nda kayma ºikayeti ile baºvurdu. Hasta komplikasyonlu kronik otitis media olarak d¸º¸n¸lerek opere edildi. Operasyon s˝ras˝nda al˝nan parÁalar˝n histopatolojik inceleme sonucu t¸berk¸loz gran¸lom geldi. Bu Áal˝ºmada orta kulak t¸berk¸lozlu bir hasta sunularak tan˝ ve tedavi yˆntemleri literat¸r eºliðinde gˆzden geÁirilmiºtir. It is hard to diagnose because it can be confused with other middle ear diseases. Tuberculous otitis media should be kept in mind when middle ear infections do not respond to standard medications. Early diagnosis and treatment prevents the complications and limits the disease. In early stages, antituberculous therapy is effective and surgery rarely needed. A 47 year old man attended our department with a persistant left ear otorhea that did not respond to local and systemic antibiotics, hearing loss, and a ten day long left facial paralysis. The patient was thought to be complicated chronic otitis media and operated. The hystopathologic examination of the materials taken from the middle ear in the operation was reported as ìtuberculosis granulomaî. We reported a case of tuberculous otitis media and discussed the diagnosis and treatment procedures by reviewing the literature. Key words: Tuberculous Otitis Media, Middle Ear Infection, Hearing Loss
