3. KEZ SUBARAKNOİD KANAMA NEDENİYLE YOĞUN BAKIM YATIŞI OLAN FİZİKSEL İSTİSMAR - OLGU SUNUMU

Amaç: Çocuk istismarı; çocuğun fiziksel, cinsel veya ruhsal olarak travmaya uğramasına sebep olmaktadır. Olgumuz üzerinden istismara uğrayan çocukların muayene bulgularınının tartışılması ve erken tanınmasının öneminin vurgulanması amaçlanmıştır. Gereç-Yöntem: Çocuk istismarı şüphesi nedeniyle adli tıp birimimize konsultasyon ile danışılan olgunun muayene bulguları ve görüntüleme tetkikleri üzerinden olgu tartışılmıştır. Bulgular: Çocuk yoğun bakımında ensafalit tanısıyla ilk yatışından bir hafta sonra nöbet hikayesiyle çocuk acile getirilen 2 yaşında kız olgunun Beyin BT’sinde subdural kanama tespit edilmiş, beyin cerrahi tarafından girişimsel işlem düşünülmeyen olgu takipleri sonrasında kanamanın düzelmesiyle, bilateral görme kaybı sekeliyle taburcu edilmiştir. 2. Taburculuğundan 1 hafta sonra konvulsiyon nedeniyle tekrar başvurmuş, dış merkez çocuk nörolojisine sevk edilmiş, sevk edildiği merkezden taburculuğundan 3 gün sonra başında şişlik nedeniyle tekrar çocuk acile başvurmuştur. Olgunun çekilen Beyin BT’sinde parital kemikte lineer kırık, subdural kanama tespit edilmesi sonrasında; travmatik subdural kanama, düşme, sarsılmış çocuk sendromu, ön tanılarıyla çocuk yoğun bakıma yatışı yapılmış ve konsultasyonları istenmiştir. Adli tıp konsultasyon cevabımızda; adli mercilere ve sosyal hizmetler kurumuna istismar şüphesi ile bildirim yapılması, çocuğun lezyonlarının ayrıntılı biçimde tanımlanması ve fotoğraflanması, istismar nedeniyle oluşabilecek eski kırıkların tespiti açısından grafilerinin istenmesi gerektiği bildirilmiştir. Sonuç: Türkiye’de 0-3 yaş grubu çocuklarda kaza dışı kafa travmasını bildiren bir çalışmada subaraknoid kanama, kafa kemiği kırığı, subdural kanama bulguları ile gelen 3 yaş altı çocuk vakaların %7’si olası istismar, %22’si istismar olarak belirlenmiştir. Klinisyenlerin çocuk istismarı olabilecek olguların risk faktörlerini ve çocuk istismarı ile oluşabilecek lezyonları iyi bilmeleri durumunda bu olguların er ken tanı alması sayesinde daha fazla fiziksel ve ruhsal zararlar ile sakatlık hatta ölümün meydana gelmesini engelleyebileceğini düşünmekteyiz. PHYSICAL ABUSE WITH INTENSIVE CARE INSTALLED FOR THE THIRD TIME SUBARACHNOID BLEEDING – CASE REPORT 

Background: Child abuse; causes the child to be physically, sexually or spiritually traumatized. In our case, it is aimed to discuss the examination findings of abused children and to emphasize the importance of early recognition. Methods: The patient was consulted to our forensic medicine unit due to the suspicion of child abuse, and the case was discussed over the examination findings and imaging studies. Results: Subdural hemorrhage was detected in the brain tomography of a two-year-old girl who was brought to the pediatric emergency department with a history of seizure one week after her first hospitalization in the pediatric intensive care unit with the diagnosis of encephalitis. The patient was discharged with sequelae of bilateral vision loss after the recovery of the bleeding after the neurosurgery did not consider an interventional procedure. She applied again one week after her second discharge due to convulsions, was referred to the pediatric neurology of the external center, and applied to the pediatric emergency department again three days after she was discharged from the center to which she was referred, due to swelling in her head. After a linear fracture of the parital bone and a subdural hemorrhage were detected in the brain tomography of the case; she was admitted to the pediatric intensive care unit with the preliminary diagnoses of traumatic subdural hemorrhage, fall, shaken child syndrome, and consultations were requested. In our forensic medicine consultation response; It has been reported that it is necessary to report to the judicial authorities and social services institution on suspicion of abuse, to describe and photograph the child's lesions in detail, and to request X-rays in order to detect old fractures that may occur due to abuse. Conclusion: In a study reporting non-accidental head trauma in children aged 0-3 in Turkey, 7% of the cases in children under the age of 3 presenting with subarachnoid hemorrhage, skull fracture, and subdural hemorrhage findings were determined as possible abuse and 22% as abuse. We think that if clinicians are well aware of the risk factors of cases that may be child abuse and the lesions that may occur with child abuse, early diagnosis of these cases can prevent further physical and mental harm, disability and even death.
