Rare central ossifying fibroma of the mandible: a case report

Abstract

Objective: Central ossifying fibroma (COF) is a rare, benign odontogenic tumor that most commonly affects the mandible. The condition is characterized by its typically asymptomatic nature, often being incidentally detected during routine dental imaging procedures. This case report presented the clinical and radiological features of a mandibular COF. Methods: A 33-year-old female patient was referred to the Department of Dentomaxillofacial Radiology by her dentist for evaluation of a mandibular lesion. Panoramic radiography revealed a well-defined unilocular radiolucent lesion located in the left posterior region of the mandible. The patient was medically healthy, and both extraoral and intraoral clinical examinations showed no pathological findings. To obtain a more detailed radiological assessment, cone-beam computed tomography (CBCT) was performed. Results: CBCT imaging revealed a radiopaque, well-defined, unilocular, mixed lesion, measuring approximately 10 × 22 × 20 mm, extending from the apex of tooth #36 to the mesial aspect of tooth #38. The lesion exhibited a round shape with thin corticated borders, buccolingual expansion, and root resorption involving the mandibular first and second molars. The patient was referred to the Department of Dentomaxillofacial Surgery. Under local anesthesia, the lesion was surgically excised and submitted for histopathological evaluation. Based on the histopathological findings, the lesion was diagnosed as a COF. Conclusions: Odontogenic fibromas are often associated with expansile osteolytic areas, which may radiographically mimic cystic lesions, other odontogenic tumors, or osseous pathologies of the alveolar bone. Accurate diagnosis requires a comprehensive evaluation incorporating radiographic, clinical, and histopathological findings.
