Digital Model-Assisted Prophylactic Stabilization for Mandibular Preservation in Recurrent Juvenile Ossifying Fibroma

Özet

A 22-year-old woman with recurrent psammomatoid-type juvenile ossifying fibroma of the anterior mandible was treated using a mandibular preservation strategy combining local tumor resection with digital model-assisted prophylactic stabilization. Cone-beam computed tomography showed a well-demarcated expansile lesion with marked cortical thinning, indicating that resection alone could leave the residual mandible mechanically vulnerable. Preoperative imaging data were converted into a 1:1 stereolithographic mandibular model for reconstruction plate precontouring, screw-position planning, and estimation of drilling depth. The lesion was removed intraorally with an individualized peripheral bony margin, and a precontoured reconstruction plate was placed through a submandibular approach to reinforce the weakened mandibular segment. At 2-year follow-up, mandibular continuity and facial contour were preserved, with no recurrence, pathologic fracture, or plate-related complication.
