PP-142 Turk J Vasc Surg 2025;34(Suppl 1) DOI: 10.9739/tjvs.2025.06.11 Chronic type A dissections and acute-on-chronic “Triple lumen”: Challenging case Tamer Cebe, Kubra Gozacik Karakoc, Anil Akbas SBU Batman Training and Research Hospital, Batman, Türkiye Abstract Aim: Chronic type A aortic dissections are rare, and the development of an acute-on-chronic component resulting in a triple-lumen anatomy is extremely uncommon, occurring in approximately 1% of cases. Such presentations pose substantial diagnostic and surgical challenges, demanding timely recognition and intervention. This report combines current literature with our experience managing a complex case. Material and Methods: Recent literature from the past five years was reviewed to assess diagnosis, imaging, and management strategies for chronic type A dissections. Emphasis was placed on the role of CT angiography, echocardiography, and individualized treatment planning. A case from our center featuring an acute-on-chronic triple-lumen dissection was presented, highlighting surgical approach and outcome. Results: Literature demonstrates that timely surgical intervention significantly improves survival in chronic type A dissections, although technical complexity influences perioperative morbidity and mortality. In our case, urgent surgery was successfully performed, with no major postoperative complications. Conclusion: Acute-on-chronic triple-lumen presentations, despite their rarity, require prompt recognition and rapid surgical decisionmaking. Regular imaging follow-up and early intervention remain critical for reducing mortality in these high-risk patients. This case underscores the importance of multidisciplinary management and the surgical team’s role in achieving successful outcomes in complex chronic dissection scenarios.
